Vincon Diagnostic Center is committed to making your appointment easy and convenient. We have included all forms which can be completed in advance prior to your visit, which greatly expedites your check-in.Please have these items ready at check-in.

v  A valid, active health insurance card.

v  A government-issued photo I.D. (e.g., driver’s license, military card, passport, etc.).

v  A prescription for diagnostic testing by a licensed healthcare provider.

v  Payment for copayments and deductibles as specified by your insurance plan. Cash, credit cards, and money orders are accepted. We cannot accept bank checks at this time.
